Vital Signs: Temperature 99.5°F, Blood Pressure 160/92 mmHg, Heart Rate 100 beats/min, Respiratory Rate 14 breaths/min
General Appearance: Lethargic, unable to answer questions
HEENT: Normal conjunctiva, no scleral icterus, clear oropharynx, moist mucous membranes
Lymph: No cervical lymphadenopathy
Cardiovascular: tachycardic, regular rhythm, no rubs/murmurs/gallopsstethoscope
Pulmonary: normal respiratory effort, clear lung fields bilaterally, no wheezes, rhonchi, or rales
Abdomnal: soft, non-distended, non-tender
Extremities: no peripheral edema in extremities, extremities warm, no cyanosis
Neurologic: Arousable to sternal rub, not following commands
Skin: Warm, dry, no rash
