Vital Signs: Temperature 99°F, Blood Pressure 105/62 mmHg, Heart Rate 102 beats/min, Respiratory Rate 28 breaths/min
General Appearance: Lethargic, lying on stretcher
HEENT: Pupils equal round and reactive to light, mucous membranes moist
Lymph: No lymphadenopathy
Cardiovascular: No JVD, regular rhythm, no rubs/murmurs/gallopsstethoscope
Pulmonary: tachypneic, lungs clear to auscultation bilaterally
Abdominal: Soft, mildly tender diffusely without rebound or guarding, nondistended, normoactive bowel sounds, no hepatosplenomegaly
Extremities:  no peripheral edema, no clubbing or cyanosis
Neurologic: Stuporous, not following commands, moving all extremities spontaneously
Skin: no rash, warm, and dry
