Vital Signs:Temperature 99℉, Blood Pressure 110/65mmHg, Heart Rate 105 beats/min, Respiratory Rate 24 breaths/min
General Appearance: cachectic, intubated, sedated
HEENT: Normal conjunctiva, no scleral icterus, moist mucous membranes, clear oropharynx; endotracheal tube and nasogastric tube in place
Lymph: No cervical lymphadenopathy
Cardiovascular: No JVD, regular rhythm, no rubs/murmurs/gallopsstethoscope
Pulmonary: lungs clear to auscultation bilaterally
Abdominal: Soft, grimaces slightly to diffuse abdominal palpation, nondistended, normoactive bowel sounds, no hepatosplenomegaly
Extremities:  LLE edematous with 1 cm healing ulceration
Genitourinary: Foley in place, draining green urine
Skin: no rash, warm, and dry
