History of Present Illness (HPI)
A 58 year old woman with chronic kidney disease stage III (last creatinine 1.6 mg/dL) diabetes mellitus, and hypertension was discharged 2 weeks ago after admission for a lower extremity cellulitis, and presents again to your hospital. She tells you that the rash on her right leg is healing, but she has noticed some swelling in her legs and exertional dyspnea.
She reports adherence with her prescribed medications, which include cephalexin (started during prior admission), metformin, nifedipine, lisinopril, and chlorthalidone. Her diabetes and hypertension have been well controlled with no recent changes in her medication regimen. Since discharge, she has been recovering at home and returned to work last week.
