Vital Signs: Temperature 99°F, Blood Pressure 88/56 mmHg, Heart Rate 110 beats/min, Respiratory Rate 18 breaths/min

General Appearance: In no acute distress
HEENT: Normal conjunctiva, sclera icteric, mucus membranes moist
Lymph: No cervical lymphadenopathy
Cardiovascular: Regular rhythm, no murmur, rub, or gallop
stethoscopePulmonary: Clear breath sounds bilaterally, no wheezes, rhonchi, or rales
Abdominal: Tense ascites, mild tenderness to palpation over entire abdomen, no rebound or guarding
Extremities: Trace peripheral edema in extremities, extremities warm with intact pulses, no cyanosis, skin findings as below
Neurologic: Lethargic but arouseable, does not answer questions 
Skin: Jaundice, warm, dry
