History of Present Illness (HPI)

A 25 year-old woman presents to clinic with dysuria, urinary frequency, and left flank pain. She has been treated for four episodes of urinary tract infections in the past six months. She reports the symptoms are similar to her prior episodes. She does not have fevers, chills, nausea, vomiting, abdominal pain, diarrhea, or constipation. She has not noticed any hematuria.

She has no other significant past medical or surgical history and is currently on no medications. She has completed all of her antibiotic courses for her prior urinary tract infections. She is not currently sexually active. She does not use tobacco, alcohol, or illicit drug use.
