History of Present Illness (HPI)

You arrive at the outpatient dialysis unit on a Friday evening and begin rounding on your dialysis patients. There is a new patient here today, a 62 year old man who has been on in-center hemodialysis (three times a week) for 5 years. He recently moved and now will be coming to your unit.

His medical history includes hypertension, well-controlled diabetes, secondary hyperparathyroidism, anemia, and myocardial infarction a few years ago. He quit smoking last year and does not drink alcohol or use illicit substances.

He reports that his dialysis treatments have been uneventful and he regularly comes to all treatments. He is currently undergoing evaluation for kidney transplantation.
