History of Present Illness (HPI)

A 25 year-old man with no significant PMH (last seen by his primary care physician about 1 month ago) presents to the emergency department with visual changes for a few days. He tells you his vision is blurry. He also complains of intermittent headaches for the past 2 weeks. He has taken only acetaminophen as needed and takes no other medications, over the counter pills, or supplements. 

He does not report tingling, numbness, weakness, facial droop, slurred speech, dizziness, or fall. He does not have chest pain, shortness of breath, nausea, vomiting, abdominal pain, or urinary symptoms (frequency, hematuria, dysuria). He does not report illicit drug, tobacco, or alcohol use.
